Case Study: A teenaged couple, Wendell and Carmen, were in a car accident in an older vehicle which lacked air bags. The driver, Wendell, who was wearing a seatbelt, struck his head against the steering wheel and then rebounded against the seat behind him. Carmen was also wearing a seatbelt, but was turned sideways at the time of impact, which caused her head to whip around and strike the passenger side window. Ten minutes after the accident paramedics noted that Wendell was confused and unable to remember what caused the accident, although people helping at the scene report that he did not appear to lose consciousness; Carmen was unconscious. Thirty minutes later in the emergency department Wendell was more aware of his surroundings and able to recount what happened after the accident. He was diagnosed with concussion. Carmen finally regained consciousness two days after the accident. She was diagnosed with diffuse axonal injury (DAI).
